Dabigatran etexilate is the non-proprietary name for an anticoagulant medication. It's usually used in certain scenarios to prevent or treat blood clots.

Patients usually buy dabigatran etexilate under its brand name, Pradaxa. It may be sold under other names depending on location.

Understanding Dabigatran Etexilate

This product is an anticoagulant, which works similarly to warfarin. Dabigatran etexilate requires fewer blood tests and, in studies, produced fewer life-threatening bleeding events. However, it did produce slightly more gastrointestinal bleeding events; those with a history of ulcers or gastrointestinal bleeding may not want to buy dabigatran etexilate.

Dabigatran etexilate does have an antidote, idarucizumab, which is effective at countering dabigatran etexilate within moments of administration.

Patients who have a prosthetic heart valve should not buy dabigatran etexilate. Patients over 75 years of age should use it with caution; they're at increased risk of serious side effects.

Patients who buy dabigatran etexilate may want to carry an ID bracelet or similar notifications that the medication is being taken. If undergoing any sort of surgery---including dental surgery---dabigatran etexilate use will likely need to stop temporarily, or the surgery be delayed.

An important factor in determining dosage of dabigatran etexilate is kidney function. Patients will need to undergo kidney function testing, most likely a creatinine clearance (CrCl) test, prior to determining dosage. Those with severe kidney impairment will required modified dosage, or may be told to not buy dabigatran etexilate at all.

How to Take Dabigatran Etexilate

Dabigatran etexilate comes in tablets, which must swallowed whole with a full glass of fluid. Food is optional.

The medication comes in three dosage strengths:

• 75 mg
• 110 mg
• 150 mg

The correct dosage will depend largely on kidney functions. Patients with moderate to good kidney function---CrCl greater than 30 ml/min---are usually prescribed:

• Following hip replacement surgery - 110 mg for the first day, then 220 mg once daily for 28 to 35 days.
• Preventing recurring blood clots - too variable to summarize.
• Treating existing blood clots - 150 mg taken twice per day (total of 300 mg per day).
• Reduce risk of stroke in non-valvular arterial fibrillation - 150 mg twice per day (total of 300 mg per day).

Those with severe kidney damage---CrCl less than 30 ml/min---may be given lower doses of 75 mg twice per day, or told to avoid the product altogether.

Missed doses should be taken as soon as remembered unless there are 6 hours or less before the next dose, in which case it can be skipped entirely. Never take a double-dose of dabigatran etexilate to compensate for a missed dose.

Duration of treatment is variable; patients may buy dabigatran etexilate for months or years, depending on a wide range of personal variables.

Side Effects

The majority of patients who use dabigatran etexilate experience only mild gastrointestinal upset, including heartburn, indigestion, and nausea. Patients who are elderly or who have a history of gastrointestinal bleeding are more prone to serious side effects, which include:

• Severe lightheadedness
• Severe or persistant headache
• Blood in stools
• Blood in urine
• Coughing or vomiting blood

The above warrant medical attention.

Dabigatran etexilate is known to interact with certain other medications. Of greatest concern are:

• Dronedarone
• Rifampin
• Ketoconazole
• NSAIDs, including over-the-counter varieties
